• Hi, My name is Diane Kelley and I am an experienced Licensed Clinical Social Worker and Certified Clinical Trauma Professional. I received my Bachelors of Social Work from Long Island University and my Master of Social work from New York University. I have worked in Mental Health Outpatient Clinics, Community Centers, Recovery programs and private practice dealing with a large variety of mental health issues, co-occurring and substance use issues.

On the outside, you may look like you have everything together. You work hard, provide for your family, and carry responsibilities well. Yet internally, you may feel overwhelmed, disconnected, anxious, angry, or caught in the same patterns despite your best efforts to change. My practice is built for people who are ready to move beyond simply talking about their problems. Together, we’ll identify the underlying beliefs, emotional patterns, and coping strategies that keep you feeling stuck. I am a board-certified Psychiatric Mental Health Nurse Practitioner with more than 20 years of nursing experience. I earned my Master of Science in Nursing from Walden University and have worked in both outpatient and inpatient settings, including Nicklaus Children’s Hospital, Jackson Memorial Hospital, and Baptist Health South Florida. I provide psychiatric evaluations, medication management, psychotherapy, and crisis intervention for children, adolescents, adults, and older adults. I treat conditions including anxiety, depression, mood disorders, ADHD, trauma, and substance use. As a bilingual provider in English and Spanish, I am committed to making compassionate mental health care more accessible.

I am a Licensed Mental Health Counselor in the state of Florida and the founder of Self Creation Holistic Counseling & Wellness. I earned my Master's degree in Clinical Mental Health Counseling from Nova Southeastern University and have had the privilege of working with children, adolescents, and adults from diverse backgrounds. I have supported individuals experiencing anxiety, depression, trauma, relationship difficulties, life transitions, grief, low self-esteem, and substance use concerns.

I’m Tiffany Ward, LMHC, CBT-C, a Licensed Mental Health Counselor and Certified Cognitive Behavioral Therapy Clinician. I help clients navigate anxiety, depression, trauma, mood-related concerns, life transitions, stress, relationship challenges, and emotional difficulties.

As a Certified CBT Clinician (CBT-C), I specialize in helping clients identify and challenge unhelpful thought patterns, understand how their thoughts impact their emotions and behaviors, and develop practical skills that support lasting change. My approach is supportive, direct, and empowering, using evidence-based interventions including Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) to strengthen emotional regulation, coping skills, confidence, and overall well-being.

As a former Division I collegiate athlete, I also understand the unique pressures associated with athletics and performance. I enjoy supporting athletes and high-performing individuals experiencing performance anxiety, confidence concerns, stress, perfectionism, and the pressure to succeed.

For clients who desire it, I offer Christian counseling that integrates faith, biblical principles, and evidence-based therapeutic interventions. Faith integration is always based on each client’s preference and comfort level.
