Hi, I’m Hannah! Born and raised in North Carolina and proud graduate of the University of North Carolina Wilmington, where I earned my Master’s in Social Work in 2022 and in VA undergraduate degree in Psychology in 2018.

This is a telehealth practice. As a personal choice, the use of narcotics or other controlled (DEA) meds are minimal, if any. I've been a psych nurse practitioner for at least 14yrs. I believe in managing conditions with psychiatric and non-psychiatric treatment modalities that are evidence based. The use of the least amount of meds, at lower doses that does the healing of symptoms, is an active process involving the client and based on the reported experiences. Safety is the core of my practice and my job is to help clients understand internal signals and how to prevent another crisis.

I am a board certified Psychiatric Nurse Practitioner that has a passion for mental health. I believe that everyone should have access to high quality care when it comes to mental disorders. I specialize in adults who struggle with ADHD, anxiety, depression, PTSD, and mood disorders.
Grace has been providing mental health care in Nashville since 2017. She spent much of her early career at Vanderbilt Psychiatric Hospital, where she gained extensive experience caring for individuals across the lifespan with a wide range of mental health concerns. She has since transitioned to outpatient practice, where she provides medication management and compassionate, individualized care to adults across Tennessee through telehealth. Grace takes a warm, collaborative approach to care and believes in taking the time to understand what is really contributing to a person’s symptoms rather than simply treating the diagnosis. She works with clients to find thoughtful, practical treatment options that fit their needs, preferences, and goals.
Percy Ford, MC, LPC-MHSP established Lighthouse Correctional Counseling and Mental Health Services in 2012 to provide Individual Psychotherapy, Family Counseling, Group Therapy and Couples Counseling to offenders and their families. Mr. Ford earned a BS in Social Work from Tuskegee University in 1997 and graduated from Argosy University School of Psychology and Behavioral Sciences in 2007 with a MA in Professional Counseling.
